Community Support Team Manager (EBCAP0148)

East Bay Center a division of East Bay Community Action Program is seeking a full time (40 hours) Community Support Team Manager for our Barrington location. The Community Support Team Manager will direct multidisciplinary clinical and rehabilitative staff engaged in the coordination of health home activities and the delivery of intensive community-based services to an adult population living with severe and persistent mental illness and/or addictions.

Job Responsibilities:

  • Participate in daily organizational meetings to monitor clients’ clinical status and response to treatment, rehabilitation and support services; schedule regular meetings with individual staff members to review cases, evaluate performance and give feedback, regularly review required staff documentation (i.e. progress notes, assessments, DLA, CSP determination forms, treatment plans, and reviews).
  • Direct the day-to-day clinical operations of the team including scheduling staff work hours to ensure appropriate for day, weekend, and on-call hours; lead daily organizational staff meeting and treatment planning meetings, continuously evaluate the status of clients and conduct appropriate planning and coordination of treatment and health home activities to ensure attention to their changing needs.
  • Provide leadership and direction regarding the development of health services plans of the treatment plan meetings; Provide direction to staff regarding individual cases and supervise members of health home team in the development of wellness and prevention initiatives and health education groups.
  • Monitor client’s Medicaid flex status to ensure insurance coverage for services. Assist and train staff to assist clients with benefit needs.
  • Provide supervision to staff (individual and joint) regularly per BHDDH regulations.
  • Ensure team coverage through all hours of operations. Participate in on-call rotation providing back up to after-hours emergency services if needed for clients. This on call responsibility is exclusively by telephone.

Qualifications:

  • Master’s Degree in Human Service or a Nurse (minimum of BA degree required).
  • Three (3) to five (5) years post-graduate clinical experience in rehabilitative mental health.
  • Familiarity with IHH regulations a plus.
  • Must possess a valid driver’s license/automobile insurance, due to the amount of community travel.
