NURSE, RN - Newport (EBCAP0125)

 East Bay Community Action Program is seeking a Nurse, RN for our Health Center in Newport. The Nurse, RN is a full time, 40 hour per week position with an occasional evening and Saturday. East Bay Family Health Care is a Patient Centered Medical Home (PCMH) following recognition by the National Committee on Quality Assurance. The Registered Nurse will work with integrated PCMH Care Teams in a primary care setting. Members of the Care Teams include medical assistants, health advocates, registered nurses, medical providers, and registered nurse care managers, medical receptionists and SBIRT /other behavioral health clinicians.

The Registered Nurse works well as part of a team approach to care, attends professional advancement and educational meetings and seminars and seeks innovative ways to assist patients with their primary care needs, including wellness and preventive screenings and chronic disease management. The Registered Nurse is a professional who is responsible for working under the direction of the site Nurse Manager to providing triage and prioritize all aspects of the delivery of integrated primary care. The Registered Nurse understands East Bay Family Health Care is committed to quality improvement and that innovations and changes that may occur in the delivery of integrated primary care lead to enhanced health and wellness.

Position Summary:

  • Provide respectful, compassionate and intelligent nursing care with dignity to a predominately underserved population of all ages (pre-natal and infants, children and youth and adults)
  • Provide leadership and assistance to other members of the Care Teams • Triage communications (phone/electronic/in person) with patients after completion of a Triage Competency Training
  • Be proficient in all aspects of documentation in the electronic medical record
  • Learn to generate electronic medical reports by Care Team for clinical quality measures and implement actions within the Care Team when improvements are needed
  • Be competent and comfortable in family planning counseling and learn about the delivery of care to Title X patients


  • BSN preferred; licensed RN, State of Rhode Island.
  • Two (2) years > experience in a community health setting, acute care or emergency/urgent care
