What is the Patient Portal?

The Patient Portal is secure internet access to your medical information so you may view portions of your health record or communicate with your medical provider from anywhere there is internet access.

How can it help me?

Initially you will be able send a question to your care team, but soon you will also be able to access lab results, request a prescription refill, or pay your bill online. In the future we will open the portal to other activities; we will keep you informed of changes.

How do I sign up?

Provide the front desk with an e-mail address and pick up our Patient Portal packet. When we receive your authorization to join the Portal, we will send you a temporary password to allow you to access the Portal, at which time you can change your password. 

Can I sign up my elder parent?

Yes, with their permission. You will need to follow the same steps to sign your parent up as you will to sign yourself up. 

Can I sign up my child?

No. Children under the age of 18 years of age are not eligible. Anyone 18 years of age or older may sign themselves up.

What do I need to sign up?

You need to have an email account that you check regularly. Give the Front Desk personnel your e-mail address and state your intent to be included on the Portal. You will receive an e-mail with the link to the Patient Portal sign-up site. Then you will need to follow the prompts which will guide you through the process. You will need to choose and remember a password for yourself as you will be using it each time you log into the site. We suggest you keep your password in a secure location, because the staff at EBCAP will not be able to look up your password. 

Can I use it with my phone?

Yes, as long as you have a phone with internet accessibility.I forgot my password. What do I do?Please click “Don't remember your user name or password?” on the portal login page. You will then be guided through the password retrieval process. 

How will I know that my privacy is being respected?

The Patient Portal is located on a secure server, and the direct connection between your computer and the server is encrypted and secured. For more information, please review the Privacy Policy on the Patient Portal login page.

What is my Care Team?

Your Care Team consists of your Primary Care Physician or Nurse Practitioner, along with the Nurse and Medical Assistant that work with them. Your Care Team works together to prepare for your appointment, help arrange referrals for you, follow up on lab results, and lots of other activities to ensure you receive the best care possible.

Can I contact my doctor through the portal?

Yes. You can send a secure message through the Patient Portal that will be received by your Primary Care Physician’s or Nurse Practitioner’s Care Team. You can also send a message to another member of your Care Team like your Nurse or Medical Assistant.

What information will be available to me?

Initially you will be able to receive messages from your Primary Care Physician or Nurse Practitioner, or any member of your Care Team. Soon, you will also be able to receive lab results, ask for refills for your current prescriptions, electronically request and receive your Health Record, pay your bill online, request an appointment and other time-saving activities. We’ll let you know as these new features are rolled out.
