East Bay Health Care, a program of East Bay Community Action Program, is a patient-centered medical home that has a team of caring providers and staff that work closely together to help our patients live happy, healthy lives. We strongly believe in an integrated health care approach and are excited to collaborate with Newport Mental Health in order to provide primary care services at 65 Valley Rd., Middletown. Your primary care team will work closely with behavioral health staff at Newport Mental Health to coordinate all your medical and behavioral health care needs.

Primary care services offered at this center are for adult and adolescents ages 13+ and include preventive care, chronic disease management, acute sick visits and nurse care management. Behavioral health care services at this location are offered through Newport Mental Health.
