East Bay Health Care, a program of East Bay Community Action Program, is a patient-centered medical home that has a team of caring providers and staff that work closely together to help our patients live happy, healthy lives. Primary care services offered at this center are for adult and adolescents ages 13+ and include preventive care, chronic disease management, acute sick visits and nurse care management. Behavioral health care services at this location are offered through Newport Mental Health.

Other services that are offered include: teen time walk-in daily; assistance with applying for health insurance; home visits by members of the care team; pregnancy tests; immunizations for infants, children and adults; care for acute illness; health screenings; transgender health services, and general community education referrals.
