Getting Started with the Portal

1) Register by calling the health center where you see your provider.

2) Once you have registered, download the mobile app for convenient on-the-go access to your account.

Scan the QR code to download the mobile app and access the Portal on-the-go.

You can also search “InteliChart” in either store. The app provides full access.

3) Once you have created your account and logged in to our patient portal, start exploring to see all the features that are available!

Messages – Send and receive messages from your Health Care Team concerning your health management.
Appointments – Request appointments to be scheduled or modified.
Ask a question – Ask your Care Team questions or request additional information.
Health Record – Access your health record including test results, med lists, and encounter history.
