Getting Started with the Portal
1) Register by calling the health center where you see your provider.

2) Once you have registered, download the mobile app for convenient on-the-go access to your account.

Mobile App Scan the QR code to download the mobile app and access the Portal on-the-go. You can also search “InteliChart” in either store. The app provides full access. |
3) Once you have created your account and logged in to our patient portal, start exploring to see all the features that are available!
Messages – Send and receive messages from your Health Care Team concerning your health management. | |

Ask a question – Ask your Care Team questions or request additional information. | |
Health Record – Access your health record including test results, med lists, and encounter history. |
