EBCAP delivers comprehensive Community Health Services as a pilar of patient-centered care to ensure an individual’s medical, behavioral, and social service needs are met. EBCAP providers, behavioral health clinicians, nurse care managers, peer recovery specialists, community health workers, and other support staff work together providing patients with coordinated care services that improve patient outcomes and enhance their overall health and well-being.

EBCAP also offers patients basic needs assistance. We connect individuals to vital services like health insurance enrollment, housing, transportation, food assistance, victim/survivors of crime resources, and other community-based programs.

People We Serve

Services are provided to established EBCAP patients.

Key Features

EBCAP provides:

  • Basic Needs Assistance – connecting patients to RI’s Supplemental Nutrition Assistance Program (SNAP), health insurance providers, transportation services, community resources, and EBCAP resources.
  • Patient Advocacy – coordinating patient care with EBCAP medical, behavioral health, and social services staff.
  • Advocacy and support services through EBCAP’s Victims of Crime Act (VOCA) – assisting EBCAP medical, dental, and behavioral health patients with resources for past or current, reported or unreported victims.

Monday through Friday: 8:00 A.M. to 4:30 P.M.
